Before you are able to submit a complaint we need a little more information. Please complete the form below and then press submit button.

Do not use any Personally Identifiable Information (PII) when submitting the details of your complaint. Do Not include account numbers, social security numbers, account balances, debit or credit card numbers and PINs, or any other combination of information that would allow someone other than yourself access to your accounts with the credit union. If the Division of Credit Unions needs this information, we will contact the credit union directly.
